Dear Valued Patient,
The Eye Surgery Center of Michigan, would like to assure you we are taking all necessary precautions to keep our patients, visitors and staff safe during the presence of COVID-19. Our facility is following all recommendations from the State of Michigan and the CDC, to minimize the risk of exposure and infection.
We ask if you have any symptoms, including a fever greater than or equal to 100 Degrees Fahrenheit, flu-like symptoms, cough, shortness of breath, new onset of diarrhea or migraine headache, to please contact your physician to reschedule your surgery and call your primary care physician immediately. Additionally, if you have traveled outside of the State of Michigan in the last 14 days, or have been exposed to an individual suspected or with a confirmed diagnosis of COIVD-19, to notify our staff prior to your surgery date.
In an effort to maintain social distancing, we ask that your driver remains in their vehicle while you are having surgery. If it is necessary for a driver to enter along with you, it will be limited to one individual during your visit. Anyone entering the surgery center will complete a COVID screening including a review of signs and symptoms and a temperature check. Your cooperation in following these guidelines, is vital to limit exposure during this challenging time.
